Grants and Awards
Union Hospital Foundation has implemented a new grant application
procedure for our grant awards and needs your cooperation for the
process to be effective. Below you will find a link to download the
Grant Application Form that will be required for all grant inquiries to
the Foundation from this point forward. The form must be fully
completed to be considered, including all necessary signatures.
We will also be requiring that all supporting information be submitted
to the Foundation office one week prior to the meeting of the
Foundation's Grants & Awards Committee. Dates
for the Grants & Awards Committee meetings are as follows:

Tuesday, July 16, 2013
Tuesday, October 22, 2013
Depending on the amount of the grant request submitted it may have to be
reviewed by the Foundation's Executive Director, Grants & Awards Committee, or the
Foundation's Board of Directors.
Union Hospital Foundation is now requiring the additional submission of a Grant Outcome Report following the approval of a grant. The due date of the first Grant Outcome Report is determined on a case by case basis and is indicated on the approved grant. Below you will find a link to download the Grant Outcome Report.
