Each gift to the Union Hospital |

SERVICE LEAGUE OF UNION HOSPITAL
Members of the service league help Union Hospital patients, families and the staff with activities to
supplement the Hospital through volunteer and fundraising efforts which provide necessary services
and equipment. Since its inception in 1953, the Service League has raised more than $2,000,000, which
has been donated to the hospital through the Foundation.

In order to promote the welfare of Union Hospital and to plan and implement various activities and fund-raising projects which are considered appropriate by the governing board of the hospital, we, along with the Union Hospital Foundation have created a grant application procedure to grant "wishes," or needs throughout the hospital. To submit your Wish List Grant, this form must be fully completed and must include the necessary signatures. Applications without both of these items completed will not be reviewed. The wish list application will be reviewed by a committee with recommendations to its Board of Directors for approval. The submission of a Wish List Outcome Report following approval will be determined on a case-by-case basis. If you have any questions, please do not hesitate to contact the Service League at serviceleague@uhhg.org marked to the attention of the Wish List chair. |
MARGARET CARROLL SCHOLARSHIP The Margaret Carroll Scholarship is awarded annually to children of Union Hospital Health Group employees who attend or plan to attend an Indiana school as a full-time student. Applicants are required to have a cumulative grade point average of at least 3.0 on a 4.0 scale, or the equivalent. |

CHILDREN'S CLASSIC RUN
Children's Classic Run is an annual one-mile fun run that raises funds for Union Hospital Pediatric Therapy Department. Annually, the Foundation grants $25,000, through the event, to the department to fund scholarships to more than 60 families who have children in need of physical, social and speech therapies. Children's Classic Run is scheduled for OCTOBER 26, 2013. Registration information TBA. |
